Summary
The Community Health Nurse (CHN) provides culturally appropriate, primary health care services to individuals, families, and groups in the Wunnumin, Wapekeka, and Kingfisher Lake communities. Working within an interdisciplinary team, the CHN delivers clinical care across the lifespan, supports health promotion and disease prevention, and addresses acute and chronic health needs using a cross-cultural, community-centered approach. Flexible hours, on-call responsibilities, and travel within remote communities are essential aspects of this role.
Details
Reporting to the Nurse-In-Charge, the CHN conducts comprehensive physical, emotional, and mental health assessments and develops individualized care plans in collaboration with physicians and community health staff. Responsibilities include providing direct nursing care, home visits, palliative support, infectious disease prevention, and health education, as well as maintaining accurate patient documentation and statistics. The role also involves coordinating referrals, collaborating with external healthcare professionals, and participating in professional development to maintain up-to-date clinical skills.
Candidates must be a Registered Nurse (RN) in good standing with the College of Nurses of Ontario, have at least two years of experience in First Nations or community-based nursing, and possess strong clinical skills, knowledge of Health Canada programs, and relevant certifications such as CPR, ACLS, and immunization. Familiarity with Indigenous culture, crisis intervention, counselling, and proficiency in Oji-Cree are considered assets.
