CONTINUUM OF CARE PATHWAY:

Background

One of the key deliverables for the MACC Project was to develop a continuum of care for women who use or may have used substances during pregnancy.  In the early stages of the project, an Advisory Working Group was established that collaborated to carry out the work.

Partners in the continuum indicated that they wanted the continuum of care pathway flow chart to reflect each agency’s role in serving the pregnant addicted mother and her family. MACC consulted key players to clarify roles and responsibilities in the continuum of care.  The draft continuum of care was reviewed by Advisory Working Group Executive Committee on December 18, 2014.  The Shibogama Chiefs reviewed the draft continuum of care in March 2015..
